The sponsoring dentist _______________________________________________[referred
to as "doctor"] and the fundraising organization________________________________ [referred to as "organization"].
Agree to enter into a relationship to help both entities by raising
money for the organization and by introducing new
patients to the doctor.
Both the doctor and the organization
agree to provide best efforts to make this relationship a
success.
However, both parties agree and understand that neither the
organization and/or the doctor can predict or
guarantee the financial results to either party.
The doctor has agreed to the following:
Provide a certain number of dental kits as indicated below.
Each dental kit shall contain basic dental products as
determined by the doctor. In addition, each dental
kit shall contain a gift certificate in an amount to be
determined solely by the doctor. The doctor
shall also determine the terms and conditions of the gift
certificate. Additionally, an informational brochure about the
doctor shall accompany each dental kit.
Provide the organization a Fundent Tracking
Form so that the sales of dental kits can be accounted for by
both the organization and doctor.
Honor each gift certificate presented by the supporters of
the organization at the doctor
office in accordance with the terms and conditions outlined on
the gift certificate. The doctor will provide the same
care and professional treatment that he provides all patients
that utilize his services.
